  • When you fill out your new client paperwork electronically, you will enter in your insurance information and upload a front/back pic of your card.​

     

    We will check what your co-pay or cost of treatment will be within 24 hours of your appointment. ​

     

    If you want to know the cost before booking, you can e-mail us at info@sogtherapy.com with your insurance information and date-of-birth and we'll get back to you with the estimated cost based on your insurance plan.

  • Most clients attend therapy weekly. We recommend clients seeking bi-weekly therapy to attend the first 4 sessions weekly prior to going to bi-weekly to allow for a comprehensive assessment and insight into the situations occurring. Monthly sessions are reserved for clients phasing out of treatment only. Clients who complete treatment are always welcome to return.

  • We do not accept checks or cash. 

     

    All clients must have a working debit or credit card on file in their chart. 

     

    Charges for sessions are collected automatically within 24 hours of the session taking place by charging the card on file. 

     

    Clients are not permitted to accumulate a balance of unpaid session costs.

     

    Missed session or late cancel fees will also automatically be charged to the card on file.

  • Deductible: The amount you have to pay each year before your insurance will contribute to the cost. This amount re-sets every year on Jan 1st.

     

    Copay: The amount charged to you for sessions. This amount is fixed, and is based on your plan. It's usually anywhere from $0-60 per session. 

     

    Co-insurance:  Once you hit your deductible, this is a percentage of the cost that you pay now that your insurance is also paying. This usually is around %10 of the cost of treatment (anywhere from $0-$20 per session)

     

    Out of Network: This means we do not have a contract with your insurance company, so they will not pay for service with us and you will either be self-pay or have to use your out of network benefits.

  • If you are divorced/separated, and have a joint custody agreement, then we are required to have our intake paperwork signed by both legal guardians to begin treatment.

     

    Our system will automatically e-mail each guardian separately and will send appointment reminders, documents, etc., to both parties.

     

    Please designate a card on file to be charged for the cost of treatment. 

     

    All minors under age 18 will require legal guardian approval for treatment. The age of the minor will determine the appropriateness of guardian involvement in treatment.

     

    Any associated risks/concerns of harm to self or others are legally required to be shared with legal guardian regardless of age of minor. However, session content outside of risks/concerns is unlikely to be discussed with guardians unless special circumstances have been arranged and agreed upon by guardians, the minor, and the therapist.

  • Sessions canceled without 24 hours notice, no-shows, and/or arriving 15 min late will result in the missed-session fee being automatically charged to the card on file. This applies to new clients.

     

    The missed session fee is $100.

     

    Special circumstances such as medical emergencies will not generate a missed session fee. Documentation may be required. 

     

    Cancellations that occur with 24 hours notice will not generate a fee.

     

    Frequently cancelling or missing 2 sessions in a row will disrupt the effectiveness of therapy, and will result in a discussion with your therapist about removing barriers to attending treatment, or potentially discontinuing treatment.

New clients are required to complete all paperwork and intake documents within 24 hours before the scheduled session. Failure to submit complete documents may lead to the cancellation of your session. Late cancellations and no-shows are subject to a $100 fee.

When scheduling online, select a recurring time slot that suits your schedule. Once your therapist confirms the request, they will establish your appointments as recurring. This ensures your chosen time slot is reserved for you each week.​
We recommend clients seeking bi-weekly support attend the first 4 sessions weekly to allow for proper rapport building and symptom capture. ​
